FOR A COMPLETE REGISTRATION YOU MUST BRING THE FOLLOWING ITEMS WITH YOU:
Participant and parent/guardian MUST BE PRESENT for signatures on paperwork as
well as a mandatory weight for football players and uniform fittings for cheer.
ORIGINAL and TWO copies of birth certificate ORIGINAL proof of residency (1)
A recent 2 x 2 color picture for the league to keep. This must be a headshot
with no hats or glasses. Registration fees of $200 for first player in family
and $150 for each additional participant. $85 fee for flag players. $40 late fee
for registrations after June 16. Scholarships are available. In addition, each
player MUST supply a completed physical on the official Pop Warner physical form
and a final (full year) report card for the 2011-2012 school year by July 23 and
be available for mandatory conditioning on August 1.

Tips:
**Make
sure you have the full year report card before the school offices close for the
summer ~ the offices will not re-open before the July 23rd deadline.
**All physicals must be pen to paper (no photocopies) AND only on the Pop Warner
physical form. No other form is
accepted. **Due to privacy, if you have military insurance, please do not
list the sponsor’s social security # as your policy #. You can write in ‘military’ for
policy #.
IMPORTANT DATES TO REMEMBER:
Paperwork Deadline: July 23rd
Mandatory Parent Meeting: July 26th, 6:00pm, CHS football
field/bleachers
First day of practice/conditioning: August 1st
Most teams will hold practice 5 days/week until school begins
No
refunds after Aug 10th
Mandatory Weight Certification: August 12th ; Sweetwater HS; Time TBD
(player must make weight in order to continue practicing in pads)
Expected first game: August 25th.
Season should run through mid-November
